Insurance and Payment Policy Update
I am no longer billing health insurance. I apologize for any inconvenience this may cause.
***Flexible Spending/Health Savings Accounts which use a credit card are also acceptable forms of payment.
This has been a very difficult decision as I know this may impact your ability to see me for massage. Please keep in mind many insurance plans do reimburse for out-of-network providers! Please let me know if you are in need of a sliding scale.
***I will continue to bill for PIP claims (auto accidents/personal injury claims)
If you currently use health insurance for massage…
Please be advised that many plans will reimburse for massage.
With each visit I will provide you with:
- A detailed invoice.
- Chart notes.
- A HICFA form with instructions or Superbill to submit to your insurance for reimbursement.
You will then be responsible for submitting the documents to your insurance provider.
To submit to insurance, you only need:
- Invoice/chart notes
- HICFA or Superbill
- Rx for massage (from your PCP)
*Your insurance company may say you do not need a prescription for massage. However, I strongly encourage you to be prepared. Since I cannot diagnose, you need to get a diagnostic code (CPT code) from your Primary Care Physician. This code is found on the Rx written by your PCP.
I see many clients who are successfully reimbursed for massage. You can find out the percentage of reimbursement by calling your insurance company.
These changes will allow me to put my focus on giving you my best and practicing more mindfully, while learning new techniques and developing classes to better serve you.
For those of you self-payers having difficulty scheduling, these changes will likely provide more opportunity to schedule your appointments with less notice!

Thank you in advance for your patience and understanding. Have a wonderful holiday and I look forward to seeing you in the future!
Love, Elizabeth
p.s. Gift Certificates available for massage! They are a great way to thank your sweetie for all of their hard work this year!
